| Dr. Philip W. Tally was born in Gadsden, Alabama. He attended medical school at the University of
Alabama, Birmingham and his neurosurgical training at Mayo Graduate School of Medicine, Rochester,
Minnesota. Dr. Tally is the founding partner of NeuroSpinal Associates and a past president of the Florida Neurosurgical Society. He has served in a leadership role for the "Brain Attack" series in conjunction with
Blake Medical Center and Manatee Memorial Hospital, where he is Vice-Chief of the Medical Staff. |

| Dr. Michael A. King was born in Kingston, Jamaica, West Indies. He attended medical school at Johns
Hopkins University, Baltimore. Dr. King completed his surgical and neurosurgical internship and residency at Vanderbilt University Hospital, Nashville and his pediatric neurosurgical residency at University of California,
San Francisco. Dr. King has been affiliated with NeuroSpinal Associates since 1990. Dr. King has served
on the Neuroscience Committee and the Surgical Case & Special Procedure Review Committee at Blake
Medical Center. |

| Dr. James A. Tiesi was born in Chicago, Illinois. He attended medical school at the University of Illinois and completed his residency in Neurosurgery at the University of Illinois, Michael Reese and Cook County Hospitals
in Chicago. He has extensive training in neurotrauma. Dr. Tiesi joined NeuroSpinal Associates in 1996.
Dr. Tiesi's interests include golfing, classical guitar and the study of eastern philosophies. He is the proud
father of 2 sons, Christopher and Alexander. |
